What is Gallbladder?

The gallbladder is a small pouch located just under the liver. It is responsible for storing bile produced by the liver. Following meals, the gallbladder becomes empty and flat, resembling a deflated balloon. Before a meal, it may be filled with bile and approximately the size of a small pear. Gallbladder removal, known as Laparoscopic Cholecystectomy, is a common minimally invasive surgical procedure.

Two types of Cholecystectomy:

  • Open Cholecystectomy
  • Laparoscopic Cholecystectomy (Preferred)

Types of gallstones

Types of gallstones that can form in the gallbladder include:

  • Cholesterol gallstones: Accounting for most cases, exhibit a yellowish hue and are primarily formed from cholesterol that fails to dissolve in bile. While cholesterol is the primary component, these stones may also contain other substances.
  • Pigment gallstones: Pigment gallstones, characterized by their dark brown or black colouration, develop due to an excess of bilirubin in the bile. Elevated levels of bilirubin can lead to the formation of these stones within the gallbladder.

Types of treatments

  1. Medical Treatment: Medications to dissolve gallstones Consider oral bile acid pills as an alternative to surgery for gallstone removal. This long-term treatment may take months or even years and is typically reserved for those who cannot undergo surgery.

A cholecystectomy is the surgical removal of the gallbladder. Using advanced laparoscopic technology, it is now possible to remove the gallbladder through a tiny incision at the navel. The patient receives general anaesthesia. Then a small incision is made at the navel and a thin tube carrying the video camera is inserted. The surgeon inflates the abdomen with carbon dioxide, a harmless gas, for easier viewing and to provide room for the surgery to be performed. Next, two needle-like instruments are inserted from the other two points over the abdominal surface. These instruments serve as tiny hands within the abdomen. They can pick up the gallbladder, move intestines around, and generally assist the surgeon. Several different instruments are used to clip the gallbladder artery and bile duct and to safely dissect and remove the gallbladder filled with stones. When the gallbladder is freed, it is then teased out of the tiny navel incision. The entire procedure normally takes 30 to 60 minutes. The three puncture wounds require no stitches and may leave very slight blemishes. The navel incision is barely visible.

Removing the gallbladder will stop the pain and treat the infection caused by gallstones. It can also stop gallstones from coming back. If gallstones are not treated, pain and infection can get worse. The gallbladder may burst, which can be life-threatening.

The laparoscopic procedure has several benefits:

• Quicker recovery and return to regular activities.
• Smaller wounds and scars.

Like any surgery, laparoscopic cholecystectomy has some risks, but they are rare in expert hands:
• Accidental bile leaking.
• Bleeding from surgery site
• Complications from anesthesia.
• Incision site Hernia.
• Inadvertent injury to the bile ducts, liver or intestines.
• Slight pain over the surgical area.
• Peritonitis (inflammation and infection in the abdomen)
• Small scars from the incision.

If there are no complications of the procedure, you can usually go home the same day as your laparoscopic cholecystectomy. With an open cholecystectomy, you may have to stay in the hospital for two or three days.

The removal of the gallbladder instead of just the stones provides a more definitive and long-term solution to the problem. Even if the gallstones are removed, there is a high chance of new stones forming in the future. If the gallbladder is not removed, the bile can continue to form stones over time, leading to recurrent gallstone-related problems. Furthermore, some people may have multiple small stones in the gallbladder. Removing each stone may be challenging and does not guarantee complete removal.

Medications are generally not effective in dissolving large gallstones. They are more suitable for smaller stones. Surgical intervention is often necessary for larger stones.

Passing a 1 cm gallstone naturally is unlikely due to its larger size. 

Gallbladder stone pain generally occurs in the upper right quadrant of the abdomen. However, the pain may also radiate to the back or right shoulder. The exact area of pain can vary from individual to individual, but it is commonly felt in the region just below the right ribcage.
